An Avon and Somerset Police officer is under investigation by the Independent Office for Police Conduct (IOPC) following the use of a Taser on an 18-year-old who was subsequently hit and killed by a vehicle on the M5 motorway.
The incident occurred shortly after 11pm on November 30, 2025, when Logan Smith was being transported in an ambulance northbound on the M5. Due to concerns about his wellbeing, the ambulance stopped on the hard shoulder near the Weston-super-Mare junction. Logan exited the ambulance and ran onto the motorway carriageway.
Police were called for assistance, and an officer arrived promptly to engage with Logan. As the situation unfolded, the officer discharged their Taser in an attempt to control the young man. Logan fell into a live traffic lane and was struck by a southbound car. Despite immediate medical attention at the scene, Logan tragically died.

The IOPC is investigating whether the officer’s use of the Taser was proportionate and in line with tactical protocols and training. One key focus of the inquiry is whether the officer complied with the required pre-discharge Taser check, which, according to Avon and Somerset Police protocol, is mandatory. Preliminary findings indicate the Taser operated as intended.
The watchdog has indicated that there is potential misconduct related to the officer’s decision to use the Taser and the failure to perform the pre-discharge check. Allegations of misconduct, if proven, could result in disciplinary measures such as a final written warning, but do not typically lead to dismissal. The investigation remains ongoing, and no conclusions have been made about disciplinary actions.
Derrick Campbell, Director of the IOPC, expressed sympathy for Logan’s family and emphasized the thoroughness of the independent investigation. An expert Taser report has been commissioned, and its findings are being reviewed alongside evidence and shared with all relevant parties, including the family, Avon and Somerset Police, and the coroner.
